Screening labs are done as general prevention. They are often part of an annual physical to detect problems, such as diabetes or high cholesterol. Diagnostic labs are designed to diagnose diseases based on specific symptoms. Additionally, labs can be used to monitor a disease. In general, screening labs are covered by insurance as a screening item whereas diagnostic labs and those labs used to monitor a disease vary in regard to their insurance coverage.

A physical is a preventive exam generally performed once a year. An office visit is used to address diseases or evaluate symptoms. If you came in for a physical but discussed new or ongoing diseases or symptoms, then this would be billed as an office visit. Many times this would be in addition to the charge for your physical.

Alliance, Anthem Blue Cross Blue Shield, Cigna, Coventry, Dean, FirstHealth, Group Health Cooperative of Eau Claire, HealthEOS/MultiPlan/PHCS, Health Choices, HealthPartners, Humana, Medical Associates, Medicare, Midlands Choice, Quartz, Sisco, Tricare, United Healthcare, WEA, Wisconsin Medicaid, WPS.
However, insurance companies have multiple plans. To ensure the most accurate information and to confirm network status, contact your insurance company at the number listed on the back of your insurance card.
Medicaid HMO’s accepted at HPFM:

The patient must choose an HMO with the State when applying for or renewing their coverage.
